Request a referral for a speech and language and/or occupational therapy evaluation and treatment from your pediatrician.

Request a hearing and vision screening from your pediatrician or through your child’s school.

Complete our Intake Forms and email them to us.

Provide any relevant work samples or additional information from your child’s teachers.

Evaluations are one hour for both speech and occupational therapy evaluations.

Treatment sessions for occupational therapy are one hour.

Treatment sessions for speech-language therapy are 30-45 minutes.

Cancellations must be made 24 hours before the appointment unless for sickness or an emergency. No shows result in a $25 charge.

We provide services in person at our office in 1:1 and group settings, or via teletherapy.

Coverage of these services can greatly vary between insurance policies. The first step in verifying whether your insurance policy covers speech and/or occupational therapy is to contact your insurance provider. Puzzle Piece is a preferred provider for Blue Cross Blue Shield, Tricare, and out of pocket payers.

Puzzle Piece provides speech and occupational therapy services on a weekly basis. Our speech and occupational therapists work closely with parents to determine how many sessions your child is recommended to receive a week. Therapy is typically recommended 1-2x/week. This recommendation is highly dependent on the severity of the child’s needs.

Duration of treatment is decided individually for each of our clients and can vary from short to long-term plan of cares.
